Axial Exchange: Care Teams for the Community: Patient-Provider Communication

Joanne Rohde launched Axial Exchange at Health 2.0 in 2010 and has been back a few times since to showcase their rapid growth. This year, Joanne shared the mobile patient interface that can be used for procedure education and medication adherence, and show how providers can track who’s engaging.

Bridges Between Innovators and Hospitals for Care Transition Improvement

Care transitions will be increasingly important for patient care. At the same time, hospitals are undergoing a major transformation and could benefit from innovation that Health 2.0 companies bring to the care transition improvement market. But what do hospitals know about Health 2.0 companies? How do they think about using them? And what do Health 2.0 companies know about making it easy for hospitals to work with them? With support from the Gordon & Betty Moore Foundation, Health 2.0 Advisors is doing research that will result in two exciting reports and a company database later in 2012. Come and learn more about this market and the upcoming resources

Interview with Matt Mattox

Matt Mattox, Co-Founder & VP of Products, Axial Exchange
Axial Exchange helps patients transition from hospital to home. Specifically, Axial delivers health education and organizes post-discharge services to help patients help themselves. This leads to lower readmission rates and higher patient satisfaction scores.

Axial Exchange – Developer Challenge Innovation Showcase

Matt Mattox, Axial Exchange
A web-based application that enables information to flow to a patient’s next care setting so that providers have what they need, when they need it, and to engage patients with the information and tools needed to improve their health knowledge and enhance their ownership of after-care responsibilities.  It bridges care-transition gaps across the entire care delivery chain, from first-responders and hospitals on through to providers and patients at the time of discharge.

Axial Exchange at Launch!

Joanne Rohde, the CEO and founder of Axial Echange, demonstrates how their products try to solve problems of continuity of care between care delivery at hospitals and care delivery by primary care doctors.

Launch!

To be on Launch! it must be the first time a company has demoed at a conference, and the product must either be coming out soon or have been in very limited beta to this point. During 2010 we saw some larger, better funded companies on Launch! but everyone’s had to compete to get on and there are some teeny startups here that you’ve never heard of that might just blow you away. It’s 10 continuous 4 minute demos, and the winner gets a mainstage spot the following year, not to mention bragging rights. Who wins? You decide.

Axial Exchange – The Healthcare Transition

Joanne Rohde, CEO, Axial Exchange

Joanne launched Axial Exchange at Health 2.0 last year. This year he’s presenting Axial’s open source SaaS, which extracts data across multiple EMR systems, providing it in usable bites to physicians.  Former COO and Director of Health IT Strategy of Red Hat, Joanne brings a wealth of experience to Axial’s missino to revolutionize healthcare. This demo was part of “The Doctor Will See You Now: Health 2.0 in the MD’s Office (Part 1)” panel at the Health 2.0 San Francisco 2011 conference.
